New Patients and Clients:
Your intake paperwork will be emailed to you after you schedule your initial visit.
Please fill it out prior to your appointment.
Dr. Hubka is available for appointments worldwide via phone by scheduling below.
If you are outside of Oregon, Dr. Hubka can offer educational health consultations.
However, because she is licensed only in Oregon, she cannot act as your doctor.
She is more than happy to provide you with a superbill to submit to your insurance for reimbursement.
All sales are final.
New Client Program:
When beginning our work together, I find that new clients see the most massive results when working with me for 3-4 sessions initially, which is why I have my new clients sign up for a concentrated program to start.
I care about helping my clients achieve real, tangible results and this allows me to support my clients in the deepest way possible.
After this container, if necessary we can renew this package or work together as needed.
Recommended Remedies & Supplements
The remedies your body asks for are an additional fee, based on what comes up when she muscle tests for you after each
appointment.
Remedies per month can range anywhere between $35-$250.
We will arrange payment for them, and have them shipped to you right away.
The recommended natural and holistic tools can include any of the following:
Homeopathic remedies, UNDA numbers, flower essences, counseling, lifestyle and nutrition habits, emotional healing
work, Chinese herbs, western herbs, cell salts, gemmotherapy, phyto-gens, oligotherapy, supplements, etc.

New Client Program: Initial Appointment (75 Minutes) + Three Follow-Up Appointments (60 Minutes Each) - $1750
Established Client Follow-Up Appointments: 60 minutes - $225
