New Patients and Clients:
Your intake paperwork will be emailed to you after you schedule your initial visit.
Please fill it out prior to your appointment.
Dr. Hubka is available for appointments worldwide via phone by scheduling below.
If you are outside of Oregon, Dr. Hubka can offer health consultations.
However, because she is licensed only in Oregon, she cannot act as your doctor.
She is more than happy to provide you with a superbill to submit to your insurance for reimbursement.
All sales are final.
New Client Program:
When beginning our work together, I find that new clients see the most massive results when initially working with me for several sessions, which is why I have my new clients sign up for this concentrated program to start.
I only care about helping my clients achieve real, tangible results and this allows me to support my clients in the deepest way possible.
After this container, if necessary we can renew this package, create a customized program, or work together as needed.
This program includes an Initial Appointment that is 75 minutes. Plus three Follow-Up Appointments that are 60 minutes each.
