New Patients and Clients - Your intake paperwork will be emailed to you after you schedule your initial visit.
Please fill it out prior to your appointment.
I am more than happy to provide you with a superbill to submit to your insurance for reimbursement.
Rates
Initial Appointments: 75 minutes ($350)
Follow-up Appointments: 60 minutes ($225)
